National College Health Assessment Web Survey

You may have recently been among those randomly selected to receive an e-mail from Dr. Lawrence, Medical Director of the Student Health Center, inviting your participation in the National College Health Assessment Web survey (NCHA-Web) sponsored by the American College Health Association (ACHA). The NCHA-Web is a legitimate survey and we encourage you to participate. The survey opens on October 29th and will close within three weeks. The survey is designed to assess student health behaviors in order to provide better services and support for Rensselaer Polytechnic Institute students. You may benefit by knowing that you have assisted in providing accurate information regarding health behaviors at RPI. As added incentive, 10 students will be randomly selected from among all completed surveys to receive a gift certificate at the Rensselaer Bookstore valued at $100. Your participation is completely voluntary and confidential. Neither your name nor email address will be associated with your responses; no one at RPI will ever know how you responded. You have been assigned an ID number in order for the secure Internet server to manage your online survey input. This number is imbedded in your URL address. To ensure confidentiality, all ID numbers and e-mail addresses are destroyed before summary data are compiled and shared. Data transmission is encrypted and firewall securities are in place.
