Student Health Center Requirements for First-Year, Transfer and New Graduate Students

There are four health center requirements for all new students:

1. Immunizations and Physical Exam - The Immunization Form is a paper form that your physician's office must complete and sign. All students must submit the immunization form. The deadline for completing this form is June 16th. If you are playing a varsity sport you will also need to have your physician fill out the physical exam form and submit this documentation before you will be cleared to participate. Students who are not participating in varsity athletics do not need to have a pre-entrance physical.  *Required immunizations are available at the Student Health Center for a fee. Immunization clinics will be held in early September. If you will be receiving immunizations from the Health Center when you arrive on campus, please send the partially completed Immunizaiton form now.

2. Online Health Portal Forms - Complete the four forms* on the Online Health Portal (Patient Information-Medical History; Notice of Privacy Practices; Tuberculosis Screening; Health Insurance Waiver-Enrollment). Follow this link to register for the Student Health Center Web Portal.  You must use Internet Explorer or Mozilla Firefox as your browser. Follow the directions to fill out the forms and submit them electronically.  The portal will not be available to new students until approximately 10 business days after their admissions deposit is received.

*The online portal forms include a pre-entrance Tuberculosis Screening.  For students who need to complete Tuberculosis testing in the United States,
testing is available at the Student Health Center for a fee. Tuberculosis testing clinics will be held in early September.

3. Health Insurance - All full-time students must have health insurance that will provide on-going, routine care in Troy, NY. To meet this requirement, all full-time students must either enroll in the RPI sponsored Health Insurance plan (offered by CDPHP) or waive out of the RPI/CDPHP plan by providing proof of an existing insurance plan that provides for routine, ongoing care in Troy, NY. The enrollment or waiver is completed via the Online Health Portal (one of the four mandatory forms described in #2 above). Health Insurance waiver/enrollment must be submitted by September 15th.

**New Graduate Students:
In addition to requirements 1 - 3 above, you are required to have Dental Insurance and will be automatically enrolled in the RPI/Delta Dental dental insurance plan. If you currently have dental insurance, you may be eligible to waive out of the RPI/Delta Dental plan. Please see this page for more information.

 **INTERNATIONAL STUDENTS: Travel insurance does not work the same as United States health insurance plans. Travel insurance will not meet the requirements to waive the RPI/CDPHP plan. Do not purchase a Travel Insurance Plan. You will need to enroll in the RPI/CDPHP plan by affirming enrollment on the health portal. You will only be allowed to waive if you have a traditional, US-based health insurance plan.

4. AlcoholEdu and Haven -[new freshmen and transfer students only; this requirement does not apply to Graduate or Exchange students] These mandatory on-line education courses will open on July 21, 2014. Part 1 needs to be completed by August 24, 2014. You will complete Part 2 forty-five days after you submit Part 1. Both Parts 1 and 2 are required. Failure to complete Parts 1 and 2 will result in a hold on your student account. The hold will prevent access to your grades and spring 2015 course registration which could lead to late registration fees. Click here for more details and log-in instructions.
