Student Health Center Requirements for First-Year Students

There are four health center requirements for all new students:

1. Online Portal Health Forms - Complete the three forms* on the Health Portal. Follow this link to register for the Student Health Center Web Portal.  You must use Internet Explorer or Mozilla Firefox as your browser. Follow the directions to fill out the forms and submit them electronically.  The Health Portal forms will not be available until on or around May 15th 2013,  and may not be available for students who have not yet paid a tuition deposit. Check back beginning May 15th for updates. 
   
   *The online portal forms include a pre-entrance Tuberculosis Screening.  For students who need to complete Tuberculosis testing in the United States, a walk-in clinic will be held in the Academy Hall Auditorium (on the same floor as the Student Health Center) September 3-6th, 9-11 am each day. 2. Immunizations and Physical Exam -the Immunization Form is a paper form that your physician's office must complete and sign. All students must submit the immunization form.  If you are playing a varsity sport you will also need to have your physician fill out the physical exam form and submit documentation before you will be cleared to participate. Students who are not participating in varsity athletics do not need to have a pre-entrance physical.
   
   
3. Health Insurance - you must actively enroll in or waive out of the RPI sponsored Health Insurance plan (offered by Chartis/AIG) at the Chartis website. Waiver and enrollment forms for the 2013-2014 school year will not be available until July 15th. See the Health Insurance page for more information.
   
   
4. AlcoholEdu - This mandatory online education program will be opened on July 24, 2013. DO NOT REGISTER/ENROLL in AlcoholEdu before this date. Please watch this page for updated information about the program.
